regular mail to the address on the bottom of this page.
Your request should include:
- name of recipient
- address
- telephone number
- email address
- equipment being requested
- amount being requested
- two personal references with phone numbers and addresses.
Your request will then be evaluated by the Board of Directors and the three members of the advisory committee and you will be notified as to the decision made.
